1. Aadhar Card of the Child
2. Aadhar Card of the Mother
3. Aadhar Card of the Father
4. Birth Certificate of the Child (in English)
5. Original Leaving/ Transfer Certificate from the last school attended (Grade 2 onwards) / Bonafide Certificate from the last School attended Grades (Junior KG to Grade 1)
6. Saral (UID Number) of the Child from the last school attended (Grades 2 to Grades 10)
7. Permanent Education Number of the Child from the last school attended (Grades Junior Kg to Grade 10)
8. Important Information Form issued by OIS signed by the principal of last school attended. (Junior Kg to Grade 10)
9. Academic Report Card from the last school attended with number of working days mentioned.
10. Current Proof of Residence (Index 2, Telephone Bill, Passport Copy, Electricity Bill)
11. 2 passport size photographs
